ABSTRACT

BACKGROUND: Adipose tissue accumulation in specific body compartments has been associated with diabetes, hypertension and dyslipidemia. Perirenal fat (PRF) may lead to have direct lipotoxic effects on renal function and intrarenal hydrostatic pressure. This study was undertaken to explore the association of PRF with cardiovascular risk factors and different stages of chronic kidney disease (CKD). METHODS: We studied 103 patients with CKD of different stages (1 to 5). PRF was measured by B-mode renal ultrasonography in the distal third between the cortex and the hepatic border and/or spleen. RESULTS: The PRF thickness was greater in CKD patients with impaired fasting glucose than in those with normal glucose levels (1.10 ± 0.40 cm vs. 0.85 ± 0.39 cm, P < 0.01). Patients in CKD stages 4 and 5 (glomerular filtration rate [GFR] < 30 mL/min/1.73 m²) had the highest PRF thickness. Serum triglyceride levels correlated positively with the PRF thickness; the PRF thickness was greater in patients with triglyceride levels ≥ 150 mg/dL (1.09 ± 0.40 cm vs. 0.86 ± 0.36 cm, P < 0.01). In patients with a GFR < 60 mL/min/1.73 m², uric acid levels correlated positively with the PRF thickness (P < 0.05). CONCLUSION: In CKD patients, the PRF thickness correlated significantly with metabolic risk factors that could affect kidney function.

ABSTRACT

@#The chronic indeterminate phase of Chagas’ disease is asymptomatic despite positive test results for antibodies specific to Trypanosoma cruzi. CD62P-APC (P-selectin) and PAC-1 FITC (GpIIb/IIIa) may improve diagnosis as biomarkers of platelet activity. Nine asymptomatic seropositive subjects, previously untreated, were selected from a blood bank within a year of Chagas’ disease detection, in addition to a control group of four. All subjects were evaluated by flow cytometry for CD62P, PAC-1 and CD41, and in a complementary study, by Tissue Doppler Echocardiography for isovolumic relaxation times (IVRT) and E/A ratios. The subjects were classified as positive or negative for CD62P and PAC-1 by a cut off obtained from their mean±2SD. For IVRT and E/A ratios, cut offs were obtained from the American Society of Echocardiography and the European Association of Cardiovascular Imaging recommendations. Fisher’s exact test was used for associated findings. Pre-test and post-test probability, sensitivity, specificity, positive and negative predictive values and likelihood ratios were calculated. Abnormalities were expressed as platelet hyperactivity and ventricular dysfunction in CD62P, PAC-1, IVRT and E/A ratios. CD62P appears to have greater sensitivity (0.75) and PAC-1, more accurate specificity (0.75), which may explain thrombotic events in Chagas’ disease. We recommend the use of CD62P and PAC-1 as biomarkers of platelet hyperactivity in patients in the chronic indeterminate phase of Chagas’ disease.

ABSTRACT The low prevalence of dementia described in communities is likely due to the low sensitivity of screening tests and an absence of evaluation by specialists. OBJECTIVE: To estimate the prevalence of mild cognitive impairment (MCI) and dementia in adults older than 50 years. METHODS: A two-phase, cross-sectional study was conducted by specialists to evaluate cognition and associated demographic risk factors in 1,235 independent community-dwelling adults from Bogotá. In Phase I, screening was performed using the MMSE and MoCA tests. In Phase II, after application of a comprehensive neuropsychological battery with neurologic and psychiatric evaluations, a cognitive diagnosis was established by consensus. RESULTS: The prevalence found for MCI was 34% and for dementia was 23%. MCI was associated with incomplete high school, OR=1.74 (95%CI=1.23-2.45), and with an age of 70-79 years, OR=1.93 (95%CI=1.47-2.53). A total of 73% of MCI cases were amnestic. Dementia was associated with incomplete primary education, OR=8.98 (95%CI=5.56-14.54), complete primary education, OR=6.23 (95%CI=3.70-10.47), and age older than eighty years, OR=3.49 (95%CI=2.23-5.44). CONCLUSION: The prevalence of dementia found was greater than the rates reported in previous studies. Low educational level was the main risk factor for cognitive impairment and should be considered in strategic planning for the local health system.


RESUMO A baixa prevalência de demência relatada em comunidades deve ser devida ao emprego de testes de rastreio de baixa sensibilidade e à falta da avaliação por especialistas. OBJETIVO: Estimar a prevalência de comprometimento cognitivo leve (CCL) e demência em adultos com idade superior a 50 anos. MÉTODOS: Um estudo transversal de duas fases realizado por especialistas, avaliando a cognição e os fatores de risco demográficos associados, com 1.235 adultos autônomos da comunidade em Bogotá. Em uma Fase I, foram realizados os testes de rastreio MEEM e MoCA. Na Fase II, após uma ampla bateria neuropsicológica com avaliações neurológicas e psiquiátricas, foi estabelecido um diagnóstico cognitivo por consenso. RESULTADOS: A prevalência encontrada de CCL foi de 34% e de demência, de 23%. CCL foi associado a ensino médio incompleto, OR=1,74 (IC 95%=1,23-2,45) e idade entre 70-79 anos, OR=1,93 (IC 95%=1,47-2,53). Entre os casos de CCL, 73% eram amnésticos. A demência foi associada a ensino fundamental incompleto, OR=8,98 (IC 95%=5,56-14,54), ensino fundamental completo, OR=6,23 (IC 95%=3,70-10,47) e idade superior a oitenta anos, OR=3,49 (IC 95%=2,23-5,44). CONCLUSÃO: A prevalência de demência encontrada é maior do que a relatada em estudos prévios. O baixo nível educacional foi o principal fator de risco para declínio cognitivo e deve ser considerado no planejamento estratégico do nosso sistema de saúde.

ABSTRACT

Background: Abrasive wear is one of the most common type of wear that not only affect teeth, as also dental restorations. Thus to investigate one of the etiological factors as tooth brushing procedure is clinical relevant in order to select the best material combination that may prevent damage of resin dental restoration's abrasion. Aims: This study evaluated the influence of tooth brushing on mass loss and surface roughness of direct Venus (Vs) and indirect Signum (Sg) resin composites, with and without a surface sealant, Fortify (F). Materials and Methods: Twenty‑four specimens were prepared with each resin composite, using their proprietary curing units, according to manufacturer’s instructions. All the specimens were polished and ultrasonically cleaned in distilled water for 5 minutes. Half of the specimens of each resin (n = 12) were covered with F (VsF and SgF), except for the control (C) specimens (VsC and SgC), which were not sealed. Mass loss (ML) as well as surface roughness (Ra) was measured for all the specimens. Then, the specimens were subjected to toothbrush–dentifrice abrasion, using a testing machine for 67.000 brushing strokes, in an abrasive slurry. After brushing simulation, the specimens were removed from the holder, rinsed thoroughly and blot dried with soft absorbent paper. The abrasion of the material was quantitatively determined with final measurements of ML and surface roughness, using the method described above. Results: ML data were analyzed by two‑way analysis of variance (ANOVA) and the analysis indicated that resin composites were not statistically different; however, the specimens sealed with F showed higher ML. Ra mean values of the groups VsF and SgF significantly increased. Conclusion: Tooth brushing affects mainly the roughness of the direct and indirect resin composites veneered with a sealant.

ABSTRACT

Objetivou-se avaliar in vitro a resiliência de dois condicionadores de tecido, UG - Ufigel SC (Voco) e D - Dentusoft (DMG), depois de serem submetidos a diferentes tempos de armazenamento em água destilada a 37ºC (24 horas, 14 dias, 30 dias e 180 dias). Foram confeccionadas 20 cápsulas de resina acrílica termicamente ativada, com 20 mm de diâmetro interno e 3 mm de profundidade. Os condicionadores de tecido, UG e D, foram manipulados de acordo com o fabricante, acomodados no interior das cápsulas (n = 10) e armazenados por 24 horas, 14 dias, 30 dias e 180 dias. Cada grupo foi submetido ao teste de resiliência utilizando uma ponta romba de 3 mm de diâmetro em máquina de ensaio universal (EMIC-DL 1000). Os dados foram submetidos à análise estatística two-way ANOVA e ao teste de Tukey (α = 0,05). Os valores de média para os materiais/tempo de armazenagem foram: UG/24 horas - 3,81A; D/30 dias - 5,54B; UG/14 dias - 5,58B; UG/30 dias - 5,66BC; UG/180 dias - 5,77BC; D/24 horas - 5,79BC; D/180 dias - 6,13C; D/14 dias - 6,54D. Dentusoft armazenado por 14 dias obteve valores de média de resiliência significantemente superiores em relação aos demais. Ufigel armazenado nos períodos de 1 a 14 dias apresentaram valores de resiliência menores do que D, sendo estatisticamente significante nesses períodos. Entretanto, para os períodos de avaliação de 30 e 180 dias, não se observaram diferenças significantes entre os materiais.

ABSTRACT

A prótese parcial removível (PPR) conjugada a implantes osseointegrados é um desafio na Odontologia restauradora, pois a biomecânica que envolve dois sistemas diferentes é um tanto quanto complexa. Este relato de caso expõe a instalação de uma PPR associada a dois implantes osseointegrados com encaixe do tipo ERA, fixados na região distal. Após minuciosos exames clínico, radiográfico e realização de modelos de estudo fez-se os planejamentos cirúrgico e protético. Restabeleceu-se função e estética associadas a um grande conforto e satisfação relatados pelo paciente, mas vale ressaltar que mais estudos clínicos são necessários para se ter um posicionamento mais definido em relação a esta modalidade terapêutica.

ABSTRACT

El objetivo de este estudio fue evaluar la resistencia a cizallamiento de aleaciones metálicas de Co-Cr y Ni-Cr; con un mismo tipo de cerámica, sometidos a diferentes tiempos de inmersión en el termociclaje. Sesenta especímenes fueron confeccionados de forma standarden formato cilindrico. Tres condiciones de termociclaje fueron evaluadas: sin termociclaje, 3000 ciclos (5ºC/55ºC±1) con 30s y 3.000 ciclos (5ºC/55ºC±1) con 60s de tiempo de inmersión. El ensayo de cizallamiento fue realizado en una máquina universal usando un dispositivo para concentrar la tensión en la interfase metalocerámica durante el test. La carga fue aplicada hasta que ocurra la fractura de los especimenes. La información fue estadísticamente analizada por ANOVA(two-way) y el test de Tukey (p<0,05). Los resultados no mostraron diferencia estadísticamente significante entre las combinaciones metal-porcelana. Sin embargo, ambas combinaciones metalocerámicas sometidas a 60s de tiempo de inmersión mostraron valores más bajos en comparación con los especimenes del grupo sin termociclaje. Fue concluido que el tiempo de inmersión de termociclaje de 1 minuto afectó los valores de resistencia de cizallamiento en los grupos de Ni-Cr/porcelana y Cr-Co/porcelana.

ABSTRACT

Objetivo: analisar as queixas vulvovaginais mais freqüentemente observadas no Ambulatório de Ginecologia Infanto Puberal (AGIP) do Hospital Universitário de Brasília (HUB). Métodos: foram avaliados retrospectivamente 210 prontuários de meninas, até 18 anos de idade, que procuraram o AGIP/HUB com queixas vulvovaginais. Analisaram-se as queixas mais freqüentes e os agentes etiológicos mais observados à secreção vaginal. Resultados: a idade média das 210 pacientes estudadas, até dezoito anos, foi de 12 mais ou menos 2,3 anos e as queixas mais comuns foram o corrimento vaginal e o prurido vulvar. Alterações inflamatórias inespecificas foram observadas em 147 destas pacientes (70 por cento) as quais foram tratadas com orientação a respeito de vestuário, de atividades diárias e de higiene. As outras 63 pacientes, nas quais foi observado um agente etiológico específico, necessitaram de tratamento medicamentoso apropriado. Somente em pacientes com vida sexual ativa foram observados infecções por Gardnerella vaginalis, Trichomoma vaginalis, HPV, assim como sífilis. Para estas pacientes a idade da sexarca foi de 14,1 mais ou menos 1,6 anos. Vinte pacientes apresentaram queixas vulvares concomitantemente, de fácil manuseio e orientação. Conclusão: higiene, vestuário e orientação adequada às pacientes e seus familiares são fundamentais para o controle e tratamento das queixas vulvovaginais, quase sempre dispensando o uso de antimicrobianos
